I-TOPICS Short Form
The I-TOPICS–Short Form-Shared Decision Making (I-TOPICS-SF-SDM) version was developed in collaboration with the Nederlandse Vereniging voor Klinische Geriatrie (NvKG – Dutch Association for Clinical Geriatrics) and Vilans, the national centre of expertise in long term care. The I-TOPICS-SF-SDM is to be used as an Individual Patient Reported Outcome Measure (iPROM) in the Dutch outpatient and clinical daily practice. In addition to patient reported health outcomes, the I-TOPICS-SF-SDM asks patients to prioritize which conditions or symptoms to address in the health conversation. Furthermore, by eliciting symptom- or disease-specific goals, functional goals and fundamental goals, person-centred decision making is pursued (Elwyn et al. 2020).
The I-TOPICS-SF-SDM (TOPICS-SF Samen Beslissen vragenlijst) is implemented in OnlinePROMS by Mediquest (info@mediquest.nl).
This Toolbox guides the implementation of the I-TOPICS-SF-SDM in Dutch geriatric outpatient clinics. The NVKG & Vilans support Dutch geriatric outpatient clinics during implementation (2021 – 2023). For more information: info@nvkg.nl
